Instructions to fill up the application form
- Online application form is only for ManagementSeats. For NRI seats you may please download the application form and fill up the form and send by post.
- Fill up the name as in your 10th standard Certificate. No change is allowed.
- Fields Marked with ” * ” are mandatory. Do not submit the application without filling the mandatory fields.
- Please Note that once the application is submitted, you cannot edit or modify the application.
- Upload recent passport size photograph.
- You can make the payment of the Application fee of Rs. 500/- on line using internet banking or credit/debit card through CCAvenue payment gateway. An amount of approximately Rs 20/- will be charged extra for the on line payment.
- Otherwise you can also pay the application fee of Rs. 500/- through Demand Draft Or Cheque in favor of “Principal, Lourdes Matha College of Science and Technology“, payable at Trivandrum. In this case enter the details of DD or Cheque such as Name of bank, DD Number/ Cheque Number and date of DD/Cheque in the application form.
- Take a printout of the completely filled up application form and put the signatures wherever required.
- The printed application with all relevant documents should reach the Lourdes Matha College Admission Office on or before 5th June 2018(Tentative Date).
- The Demand Draft or the Printout of Receipt you received for online payment through your email shall also be sent along with the application.
Documents to be attached along with the Application Form
Self attested copies of the following to be attached along with the application
- 10th Standard Pass Certificate for Proof of Date of Birth
- Marklist of +2 (HSE/CBSE/ISC/Others)
- KEAM Admit Card &KEAM Score Card or Score Sheet
- Community Certificate for Christians from Parish Priest
